Help with Documents, Forms & Appointments

Help with Documents, Forms & Appointments: find your document, prepare your questions, arrange the next step.
Prepare your information, review the recipient’s instructions and keep a copy of your completed document.

Choose your next step

If you are unsure which service to book, begin with the instructions from the organization requesting your document. Look for words such as affidavit, statutory declaration, certified true copy, witnessed signature, apostille or legalization. Send the office the document name and the requested action rather than guessing from a template title.

Find a document or country

Use Find a Document to browse document services. The Templates & Forms library includes guided builders, connected original PDFs and preparation guides. Search by form number, such as IMM 5409, or by a plain-language document name. For documents used overseas, search the apostille country directory.

Fill, download and reopen a form

  1. Open the builder for your document and read its instructions.
  2. Enter factual information. Leave signatures and official certification for the appointment.
  3. Review the preview and use the download button to keep a copy.
  4. For connected PDFs, open the saved file in Adobe Acrobat Reader and complete any required validation.

Your answers are not automatically saved by the builders. Download before closing the page. A connected PDF page can reopen a previously downloaded matching PDF; the rich text editor can reopen its formatted HTML download.

If a preview or download does not work

Refresh the page, enable JavaScript and try a current desktop browser. Check your Downloads folder. Some Adobe dynamic forms use guided fields instead of a browser PDF preview. You can also download the blank original and complete it in Acrobat Reader. See browser and download troubleshooting.
