Affidavits & Statutory Declarations in Mississauga

Request help with commissioning an affidavit or statutory declaration. Share the complete document and the recipient’s instructions so our office can confirm the service and appointment format.

Affidavits & Statutory Declarations in Mississauga: read the document request, prepare accurate information, review before signing.
Prepare your information, review the recipient’s instructions and keep a copy of your completed document.

Affidavit or statutory declaration: which do you need?

Both record facts that you formally confirm are true. With an affidavit, you swear an oath or make a solemn affirmation. With a statutory declaration, you formally declare the statement to be true. The organization receiving your document decides which format and wording it requires; the two should not be substituted without checking.

Notaryspace can assist with commissioning your prepared statement in Mississauga. The appointment involves confirming your identity, administering the appropriate oath, affirmation or declaration, and completing the signing section. You remain responsible for the accuracy and completeness of your statement.

Common affidavits and declarations

Student funding and OSAP

Your school or funding office may request a statement about marital status, family circumstances or another specific issue. Use its requested wording. Explore OSAP affidavit guidance.

Common-law relationships

Use the form specified by the receiving organization. For immigration purposes, complete the required official form rather than replacing it with a general letter. Open IMM 5409.

Names and identity details

A name discrepancy statement can explain differences between documents. Record each spelling accurately and bring the records that support your explanation. Prepare a name discrepancy draft.

Immigration and custodianship

Check who must sign each part and where each declaration must be made. A notarial appointment does not replace completing the immigration application. Open the IMM 5646 custodianship form.

Court and service affidavits

Bring the correct court form and every referenced exhibit. Commissioning your signature is separate from filing the document with the court or advising on your case.

Other personal or business statements

Tell us if you need a declaration concerning residence, a lost document, vehicle ownership, a translation or another matter. Suitability depends on the recipient’s instructions and the facts you need to confirm.

What to bring to your appointment

  • Your complete affidavit or declaration, with the signature and commissioner sections left unsigned.
  • Current government-issued photo identification; ask us in advance if your ID name differs from the document.
  • All exhibits and supporting documents referenced in the statement, in their intended order.
  • The recipient’s instructions, official form and any deadline.
  • Each person who must make a statement, with their own identification.

Read every paragraph before the appointment. Check names, dates, addresses and amounts. Do not guess facts, leave material blanks or sign a statement you do not understand. Tell us beforehand if you need interpretation or help accessing the appointment.

How the signing appointment works

  1. Confirm your form: obtain the recipient’s current form and complete the factual sections.
  2. Arrange the appointment: describe the statement, signers and intended use. Confirm any drafting assistance separately.
  3. Review and identify: present your identification and complete statement, including exhibits.
  4. Swear, affirm or declare: answer the required questions truthfully and sign as directed. A non-religious affirmation is available for an affidavit.
  5. Check your copy: review the completed document and keep a copy before submitting it through the recipient’s required channel.

Our Templates & Forms library can help you prepare. A general template is a starting point; use a prescribed government, court or institution form whenever one is required.

Fees and appointment options

The attestation fee is CAD $37.95. Ask our office to confirm the total for your documents and number of signatures or attestations, including applicable taxes and any additional services. Drafting, extra copies, translations, government processing and delivery may involve separate charges where requested. Confirm your quote before proceeding.

In-person appointments are available. Certain affidavits and declarations may be eligible for remote commissioning after review. Confirm that the recipient accepts that process before booking. Remote commissioning is distinct from notarization; Ontario does not permit remote notarization under the Notaries Act.

Frequently asked questions

Can I sign before arriving?

Leave the statement unsigned and follow the commissioner’s instructions. If you have already signed, tell us before the appointment so we can explain what is needed.

Can you commission a statement for someone who is absent?

The person making the statement must participate in the appointment and personally confirm its truth. Bringing another person’s identification or signed form does not replace their participation.

Does commissioning mean the facts have been independently verified?

No. You are making the statement under oath, affirmation or declaration. Commissioning does not establish the truth of every fact or guarantee the receiving organization will accept the document.

Can I amend the document after it is signed?

Do not alter a completed statement yourself. Contact our office and the recipient; corrections may require a new document and a further appointment.

What if the declaration is for another country?

Obtain the overseas recipient’s instructions first. An in-person notarial act, apostille, authentication, translation or consular legalization may be needed. See international document services.
