Statutory Declaration in Lieu of Guarantor

Confirm which application you are making

A declaration in lieu of guarantor may be required when an applicant cannot obtain an eligible guarantor. Canadian passport and non-Canadian travel-document applications use different forms and instructions. Identify your application before downloading or completing a declaration.

Obtain the correct official form

Use the Passport Program’s references and guarantors instructions to determine the required form and how to obtain it. Do not substitute a non-Canadian form for a Canadian passport application simply because both documents refer to a guarantor.

Prepare the factual information

  • Your legal name and the application details requested on the form.
  • The reason you cannot obtain an eligible guarantor.
  • The identity documents and references required by the application instructions.
  • The current declaration form and any written request from the Passport Program.

Use accurate information and complete only the applicant fields. Leave the oath, witnessed signature, official’s details and certification section for the authorized person. Confirm who can administer the particular declaration before arranging a visit.

Available online preparation

The PPTC 326 original-PDF builder is for non-Canadians applying for a travel document. Our general preparation builder helps organize information but does not replace a prescribed official form. Review each builder’s output label before using its download.

Appointment and submission

Bring the unsigned original form, government-issued photo identification and the application checklist. Confirm the appointment format and fee with the office. After signing, submit the declaration with the documents required for your particular application and keep a complete copy. Contact Notaryspace if you need help identifying the requested declaration.
