Used Vehicle Transfer Forms

Used Vehicle Transfer Forms: identify the transaction, check the vehicle details, prepare the requested form.
Prepare your information, review the recipient’s instructions and keep a copy of your completed document.

Identify the transfer you are making

A sale, a gift between family members and another change of ownership may require different documents. Start with ServiceOntario’s instructions for your transaction. A sworn statement is supporting evidence for the transfer; it is not a replacement for registration, ownership documents or other required records.

Family-gift sworn statements

Ontario provides the Sworn Statement for a Family Gift of a Used Motor Vehicle. Use the current official form and read its declarations before completing it. The tax treatment depends on the transaction meeting Ontario’s rules; signing a declaration does not itself establish an exemption.

Prepare these details

  • The legal names and addresses of the donor and recipient.
  • The vehicle identification number, year, make and model.
  • The transfer date and the relationship between the parties.
  • The ownership permit and the instructions for the particular transfer.
  • Government-issued photo identification for the people making the statement.

Check the VIN against the ownership record rather than typing it from memory. Explain any missing or inconsistent information to the office before the appointment. Do not describe a paid sale as a gift.

Leave signing for the appointment

Complete the factual fields but leave signatures, signing dates and the commissioner’s section blank until instructed. Confirm which people must attend and whether the proposed appointment format is suitable for the form. For certified copies, bring the originals to an in-person appointment.

Complete the transfer with ServiceOntario

After signing, follow ServiceOntario’s checklist for the permit, insurance, safety documentation and any applicable taxes. Requirements vary by transaction. Read Ontario’s used-vehicle guidance and contact Notaryspace to arrange assistance with the sworn statement.
